Incident Narrative
A VALVE THAT IS NORMALLY SECURED OPEN TO ALLOW FOR THERMAL EXPANSION WAS CLOSED DURING MAINTENANCE WORK FOR ISOLATION PURPOSES. UPON COMPLETION OF THE WORK THE VALVE WAS NOT RE-SECURED IN THE OPEN POSITION. THIS RESULTED IN THERMAL PRESSURE BUILD UP IN AN ISOLATED SECTION OF TERMINAL PIPING AND CAUSED A FLANGE GASKET TO LEAK.
