THE DRAIN VALVE ON THE PIG TRAP WAS LEFT OPEN THEREBY ALLOWING AN UNCONTROLLED FLOW OF OIL INTO THE SUMP VIA THE DRAIN LINE. AT THE SAME TIME THE HIGH-HIGH LEVEL ALARM ON THE SUMP FAILED CAUSING THE OIL TO OVERFILL THE SUMP.
