Incident Narrative
THIS INCIDENT INVOLVED AN INTRAPLANT HEADER, THE RELIEF VALVE IS SET @ 100 BUT BECAUSE WE SUSPECT THE VALVE FAILED DUE TO A FAULTY RELAY, THE VALVE OVERPRESSURED. THE PROBLEM TURNED OUT TO BE A SHORTED WIRE IN THE CONDUIT RUNNING FROM THE CONTROL ROOM TO THE MOTOR OPERATOR ON THE VALVE. WIRE WAS FIXED.
