Incident Narrative

THIS RELEASE WAS THE RESULT OF A CRACK THAT DEVELOPED IN THE ROOF DRAIN LINE OF A BREAKOUT TANK. THE CRACK IN THE DRAIN LINE RESULTED FROM AN EQUIPMENT FAILURE. THE EQUIPMENT THAT FAILED WAS A CHIKSAN SWIVEL JOINT IN THE DRAIN LINE WHICH SEIZED UP. WITH THE CHIKSAN JOINT SEIZED IN A FIXED POSITION, THE ROOF DRAIN LINE COULD NOT FREELY MOVE WITH THE UP AND DOWN MOVEMENT OF THE EXTERNAL FLOATING ROOF. THIS CREATED A STRESS ON THE LINE WHICH EVENTUALLY RESULTED IN A CRACK IN THE LINE NEAR THE SWIVEL JOINT. THE FAILURE WAS DETERMINED TO BE A LACK OF LUBRICATION IN THE JOINT. THE JOINT IS FACTORY LUBRICATED AND SEALED JOINT.
