Incident Narrative
AN INCORRECT VALVE WAS OPENED RESULTING IN A LINE SEGMENT OVER PRESSURE EVENT. THE LINE SEGMENT WAS SHUT DOWN AND AN INTEGRITY TEST WAS PERFORMED WHICH RESULTED IN IDENTIFYING A LEAK DUE TO THE OVER PRESSURE EVENT. THE LINE SEGMENT WAS EXCAVATED AND NEW PIPING INSTALLED AS A RESULT OF THE INTEGRITY TESTING. FINAL COSTS UPDATED ON 07/09/2013.
