Incident Narrative
"INCIDENT ORIGINALLY REPORTED TO THE NRC AS 77 STRAWFLOWER. ACTUAL ADDRESS OF EXPLOSION IS 91 STRAWFLOWER CT. A THIRD PARTY EXCAVATOR WAS INSTALLING CONDUIT VIA DIRECTIONAL DRILLING. A MAIN WAS STRUCK MULTIPLE TIMES DURING THE PULLBACK. THE RELEASE OF GAS WAS SECURED ABOUT 2 HOURS PRIOR TO THE INCIDENT. SUBSEQUENTLY, THE EXPLOSION OCCURRED. IN SECTION G3 QUESTION 14, ONLY ONE ROOT CAUSE CAN BE SELECTED; HOWEVER, OUR INVESTIGATION INDICATED 2 ROOT CAUSES; LOCATING PRACTICES INSUFFICIENT AND EXCAVATION PRACTICES INSUFFICIENT."
