Incident Narrative
A CORROSION COUPON HOLDER WAS OBSERVED TO BE LEAKING. THE VALVE ISOLATING THE COUPON HOLDER WAS CLOSED TO STOP THE LEAK. THE CAP APPEARED TO BE NOT TIGHT ENOUGH. THE CAP WAS REMOVED, O-RING WAS INSPECTED AND FOUND TO BE WITHIN SPECIFICATIONS, SO THE CAP WAS REPLACED AND TIGHTENED AND CONFIRMED TO BE HOLDING. THE PRODUCT RELEASED WAS CLEANED UP AND HANDLED IN ACCORDANCE WITH COMPANY AND STATE GUIDELINES.
