Incident Narrative
WHILE WITHDRAWING A TOR TOOL THE PLUG CAME LOOSE, RESULTING IN THE RELEASE. THE NEAREST VALVE WAS SHUT WHEN THE TOOL WAS REMOVED. IMPACTED SOIL WAS REPLACED WITH CLEAN TOP SOIL. THE IMPACTED SOIL WAS COLLECTED FOR WASTE CHARACTERIZATION AND DISPOSAL IN ACCORDANCE WITH STATE AND COMPANY GUIDELINES.
