"THE LEAK OCCURRED DURING THE ATTEMPTED INSTALLATION OF A 2' X 3/4' ELECTROFUSION TAP TEE ON TO A 2' HDPE MAIN. ALL OF THE MATERIALS AND EQUIPMENT USED DURING THE ATTEMPTED INSTALLATION WERE DESTROYED BY FIRE, LEAVING NO EVIDENCE TO EXAMINE. THE ELECTROFUSION PROCESS PROVIDED HEAT THAT APPARENTLY COMPROMISED THE PIPE."
