AT APPROXIMATELY 15:14 ON 11/11/11, A VALVE WAS LEFT CLOSED AT MANIFOLD 6 RESULTING IN A RELEASE AT UNIT 3. THE LINE PRESSURED UP, WENT THROUGH THE OPEN SUCTION VALVE AND OUT THE SEAL.
