Incident Narrative
"APPROXIMATELY 6 BBLS OF CRUDE OIL WERE RELEASED WHEN A 2 INCH SIDE VALVE ON THE SUCTION SIDE OF A PUMP FAILED TO SEAL CLOSED. ONCE THE RELEASE WAS DISCOVERED, THE ON-SITE OPERATOR IMMEDIATELY CLOSED THE PUMP'S MAINLINE SUCTION AND DISCHARGE VALVES. THE CLOSURE OF THESE VALVES ISOLATED THE RELEASE SOURCE THUS STOPPING THE RELEASE. THE 2 INCH SIDE VALVE IS IN THE PROCESS OF BEING REPLACED."
