Incident Narrative

"WHILE ON NORMAL ROUNDS, A SPLP MARCUS HOOK TANK FARM OPERATOR OBSERVED WHAT APPEARED TO BE A SHEEN ON A RAIN PUDDLE. SUPERVISION WAS NOTIFIED AND OIL ABSORBENT PADS WERE PLACED ON THE PUDDLE. FURTHER INVESTIGATION DETERMINED THERE WAS A PINHOLE LEAK AT APPROXIMATELY THE 6 O'CLOCK POSITION ON THE PIPE. A PLIDCO CLAMP WAS INSTALLED AS AN IMMEDIATE REPAIR."
