"FEBRUARY 28, 2013 AT APP. 14:29 THE MAINLINE ISOLATION VALVE AT MP 672 ON BL-02 EXPERIENCED A STEM PACKING LEAK. LOCAL PERSONAL WERE CALLED TO RESPOND TO THE SITE AND REPACKED THE VALVE STEM STOPPING THE RELEASE."
