Incident Narrative

WHILE DRIVING TO WORK A MAGELLAN EMPLOYEE IDENTIFIED A PETROLEUM ODOR THAT APPEARED TO BE COMING FROM THE MP 318.28 VALVE BOX. UPON FURTHER INSPECTION IT WAS CONFIRMED THAT THE STEM SEAL ON THE VALVE WAS LEAKING AND REQUIRED REPLACEMENT. UPON FURTHER REVIEW, IT WAS DETERMINED THAT THE ENTIRE VALVE NEEDED TO BE REPLACED SINCE THE VALVE WAS NOT EQUIPPED WITH A MEANS OF REPACKING THE STEM SEAL. A NEW VALVE WAS INSTALLED TO COMPLETE THE REPAIR. THE RELEASED PRODUCT WAS SUBSEQUENTLY RECOVERED.
