A 0.250"" O-RING WAS INSTALLED AND LATER FAILED. IT WAS DISCOVERED THAT THE O-RING SPECIFICATION IS 0.275"". THE CLOSURE INSTALLED AT THIS LOCATION HAD UNKNOWN SPECIFICATIONS AT THE TIME OF THE INCIDENT. SEAL WAS REPLACED.
