Incident Narrative
A CONTRACTOR WAS INSTALLING WATER MAIN AND STRUCK A 4"" PLASTIC GAS DISTRIBUTION MAIN WHILE EXCAVATING WITH A BACKHOE. LOCATES WERE MADE BY THE CONTRACT LOCATOR BUT THE GAS MAIN WAS MIS-LOCATED. ADEQUATE GAS PRESSURE COULD NOT BE MAINTAINED TO MAKE REPAIRS TO THE GAS DISTRIBUTION MAIN WHICH RESULTED IN THE LOSS OF GAS SERVICE TO 584 CUSTOMERS.
