Incident Narrative
LEAK LOCATION WAS FOUND USING DIRECT CURRENT VOLTAGE GRADIENT (DCVG). PIPE WAS EXCAVATED USING A HYDRO VACUUM UNIT. PIPE COVER WAS 9 FEET AT THE FAILURE LOCATION. PIPELINE WAS CLEANED AND INSPECTED. COATING DAMAGE DUE TO 3RD PARTY EXCAVATION WAS NOTED AT THE FAILURE LOCATION. A LEAK CLAMP WAS INSTALLED AND THE PIPE RECOATED.
