Incident Narrative
IT WAS DETERMINED THAT THE BURN-THROUGH LEAK WAS DUE TO EXCESSIVE HEAT APPLIED. AT THE TIME OF THE ACCIDENT THE CONTRACT WELDER AGREED TO A DOT DRUG AND ALCOHOL TEST AND WENT TO THE CLINIC WITHIN THE ESTABLISHED TIME LIMIT; HOWEVER, THE CLINIC ONLY ADMINISTERED THE DRUG TEST. UPON THE CAUSE DETERMINATION, THE WELDER'S OQ RECORDS WERE REVIEWED. THE WELDER MET ALL OF THE REQUIRED TECHNICAL QUALIFICATIONS TO WELD FOR MARATHON PIPE LINE BUT HAD NOT TAKEN THE ""ABNORMAL OPERATIONS"" PORTION OF THE OQ QUALIFICATION COURSES.
