"APPROXIMATELY 150 BBLS OF CRUDE OIL WERE RELEASED DUE TO MISALIGNMENT ON THE MANIFOLD. THE 16-INCH MANIFOLD WAS PRESSURED PAST THE SET POINT ON THE THERMAL RELIEF VALVE (TRV), CAUSING THE TRV TO RELIEVE INTO THE SUMP AND THE SUMP TO OVERFILL. THE FACILITY WAS SHUT DOWN AND THE MANIFOLD WAS REALIGNED BEFORE RESUMING NORMAL OPERATIONS. THE OPERATIONAL LINEUP HAS BEEN LOCKED OUT AND TAGGED OUT (LOTO) TO PREVENT FUTURE OCCURRENCES."
