Incident Narrative
A SANDBLASTER HOSE THAT WAS BEING USED TO REMOVE COATING ON ANOTHER NEARBY PIPELINE WAS POSITIONED NEXT TO THE 6"" LINE. A HOLE DEVELOPED IN THE COUPLER WITHOUT THE PERSON OPERATING THE SAND BLASTING HOSE BEING AWARE OF IT, AND BLASTED A PINHOLE IN THE 6"" LINE, RESULTING IN THE RELEASE.
