"DURING ROUTINE VALVE INSPECTIONS, EMPLOYEE APPROACHED VALVE AT MP 509, AND HEARD HISSING NOISE. UPON FURTHER INVESTIGATION IT WAS DISCOVERED THAT THE FLANGE ON A SPOOL OF THE DOWNSTREAM SIDE OF MP 509 VALVE WAS LEAKING. MARCH 11, 2015 GASKET WAS REPLACED WITHOUT INCIDENT."
