Incident Narrative
MAINTENANCE HAD BEEN PERFORMED ON THE MDG PROVER. WHEN THE PROVER WAS BEING REFILLED SO THAT IT COULD BE RETURNED TO OPERATION, THE LEAK OCCURRED. AN INVESTIGATION OF THE LEAK EVENT REVEALED THE O-RING ON THE PROVER DOOR HAD NOT SEATED CORRECTLY.
