Incident Narrative
ON 05/08/2015 AT APPROXIMATELY 18:40 A THREADED TUBING CONNECTION FAILED ON A PRESSURE TRANSMITTER. IT WAS DETERMINED THAT VIBRATION FROM THE BOOSTER PUMP CAUSED THE CONNECTION TO FAIL THUS ALLOWING CRUDE OIL TO LEAK. THE AREA WAS CLEANED UP AND THE TUBING WAS REPLACED AND SECURED TO PREVENT EXCESSIVE VIBRATION.
