Incident Narrative

"IN THE PROCESS OF INSTALLING A VALVE SETTING FOR A NEW PUMP STATION, A SECTION OF PIPE WAS STOPPLED AND BYPASSED IN THE HALLEY 5 ACRE YARD. THE STOPPLES HAD BEEN INSTALLED AND THE SECTION OF PIPE BETWEEN THE BYPASS WAS TAKEN OUT OF SERVICE. THE PIPE WAS CUT LOOSE AND THE WELD WAS COMPLETED ON THE UPSTREAM SIDE. AFTER THE WELD WAS MADE, A LEAK WAS DISCOVERED ON THE DOWNSTREAM STOPPLE. THE CONTROL ROOM AND THE UPSTREAM PLANTS WERE NOTIFIED AND THE PIPELINE WAS SHUT IN FOR SAFETY REASONS."
