Incident Narrative

ON 2-3-14 AT APPROXIMATELY 1300 HOURS A BP EMPLOYEE DISCOVERED A LEAK ORIGINATING FROM A PIG LAUNCHER DOOR AT THE TRI-STATES DUKE PLANT IN MOBILE, ALABAMA. THE EMPLOYEE ISOLATED THE SOURCE AND DIVERTED THE REMAINING NATURAL GAS LIQUIDS TO THE DUKE FLARE STACK TO RELIEVE PRESSURE IN THE PIG LAUNCHER. THE FAULTY O-RING GASKET WAS THEN REPLACED.
