Incident Narrative
ON 3/25/2014 AT 1200, A TECHNICIAN WAS PERFORMING A ROUTINE STATION CHECK AND DISCOVERED A PIHOLE LEAK ON A WELD OF A 1"" BYPASS PIPING. THE LEAK WAS CONFIRMED USING SNOOP. THE VOLUME RELEASED WAS VERY SMALL AND IT WAS NOT DETERMINED TO BE A REPORTABLE QUANTITY. HOWEVER, IT WAS LATER DETERMINED THE COST TO REPAIR MAY EXCEED $50,000 AND THE NRC WAS NOTIFIED IMMEDIATELY. THE LEAK WAS ISOLATED THEN PURGED AND THE DEFECT WAS GRINDED OFF.
