A GASKET WAS NOT REINSTALLED ON A SMALL DIAMETER FITTING DURING MAINTENANCE WORK, WHICH RESULTED IN AN INCOMPLETE SEAL. UPON DISCOVERY, THE GASKET WAS REINSTALLED AND FITTING WAS TIGHTENED. ALL PRODUCT WAS CONTAINED IN SECONDARY CONTAINMENT.
