Incident Narrative
MAINTENANCE CREW INSTALLED TEMPORARY BYPASS PIPING IN ORDER TO REPLACE A VALVE AT THE LOCATION. AS THE BYPASS PIPING WAS BEING PURGED WITH PRODUCT, AN EIGHT INCH FLEXITALLIC GASKET FAILED AS A RESULT OF LOOSE BOLTS FROM IMPROPER TIGHTENING. PIPING WAS ISOLATED, BLOWN DOWN AND THE GASKET REMOVED AND REPLACED BOLTS WERE TIGHTENED AND PROJECT CONTINUED.
