Incident Narrative
A SHORT SECTION OF ABOVE GROUND PIPE, DOWNSTREAM OF THE INLET SCRAPER TRAP WAS ISOLATED BETWEEN TWO MANUAL VALVES WITHOUT THERMAL RELIEF. THE PIPE SECTION OVERPRESSURED SPLITTING AT THE LONG SEAM OF THE SHORT PIPE SECTION. THE CORRECTIVE ACTION TAKEN WAS; REPLACE THE PIPE AND INSTALL THERMAL RELIEF TO AN ACTIVE SECTION OF PIPING.
