Incident Narrative
DURING A ROUTINE STATION CHECK AT DENVER JUNCTION, THE PUMP CASE TUBING VALVE CONNECTION WAS FOUND TO BE ICED OVER INDICATING A SMALL LEAK. THE VALVE CONNECTION WAS ISOLATED FROM THE PUMP, THE VALVE WAS REMOVED AND THE FITTING WAS PLUGGED TO STOP THE LEAK. IT IS BELIEVED THE FITTING STARTED LEAKING DUE TO IMPROPER INSTALLATION. THE VALVE WAS REMOVED AND THE CONNECTION PLUGGED IN ORDER TO PREVENT A REOCCURANCE.
