Incident Narrative
DURING THE ADDITION OF A TD WILLIAMSON FILTER TO AN EXISTING PIG SIGNAL A TALLGRASS EMPLOYEE ENCOUNTERED A THREADED FITTING THAT HAD BEEN DAMAGED. WHILE ATTEMPTING TO RECTIFY THE PROBLEM THE THREAD-O-RING THREAD FAILED (11:57AM) CAUSING AN OIL RELEASE THROUGH A 2"" ORIFICE FOR APPROXIMATELY 11 MINUTES BEFORE THE STATION WAS SHUT DOWN AND A CAP WAS INSTALLED.
