WHILE GRADING PARALLEL TO A MAIN, WITHIN THE TOLERANCE ZONE, A THIRD PARTY EXCAVATOR STRUCK A 1"" GAUGE TEE LOCATED ON THE SIDE OF A 12"" STEEL MAIN AND CAUSED A RELEASE OF NATURAL GAS INTO THE ATMOSPHERE.
