Incident Narrative
APPROXIMATELY 2 BBLS OF CRUDE OIL WERE RELEASED AS A RESULT OF TWO FAILED INSULATOR GASKETS. THE CAUSE OF THE RELEASE WAS DETERMINED TO BE IMPROPER INSTALLATION DURING CONSTRUCTION. BOTH GASKETS WERE REPLACED WITH A DIFFERENT TYPE OF GASKET AND VERIFIED FOR PROPER WORKING CONDITION. IN ADDITION, PLAINS OPERATORS VERIFIED ALL GASKETS WERE PROPERLY TORQUED. FURTHERMORE, PLAINS CREATED A NOTIFICATION AND AWARENESS REPORT (NAR), TO BE SENT TO ALL RELEVANT EMPLOYEES IN ORDER TO HELP PREVENT REOCCURRENCE.
