ON 1/20/2017 LOCAL OPERATING PERSONNEL NOTICED CRUDE OIL LEAKING FROM THE C1 SUMP. IT WAS DETERMINED THAT THE DRAIN VALVE WAS LEFT OPEN DURING PIGGING OPERATIONS. DUE TO NOT FOLLOWING THE LOCAL PIGGING PROCEDURE. AS A RESULT OF THE ACCIDENT THE EMPLOYEE WAS TERMINATED DUE TO NOT FOLLOWING THE PROCEDURE.
