Incident Narrative
ON 11/10/2015 AT APPROXIMATELY 14:30, LOCAL OPERATIONS WERE EMPTYING A VACUUM TRUCK INTO THE LOCAL SUMP. THE LEVEL TRANSMITTER FAILED TO RESPOND WITH THE CORRECT LEVEL ALLOWING THE SUMP TO OVERFLOW. ALL CRUDE WAS CLEANED UP WITH ON-SITE PERSONNEL AND TRANSMITTER WAS REPLACED.
