Incident Narrative
ON 2/22/2016, A CONTRACTOR NOTIFIED KINDER MORGAN LOCAL OPERATIONS PERSONNEL THAT THERE WAS A LEAK AT THE YELLOW JACKET STATION. OPERATIONS PERSONNEL SECURED THE STATION, AND DETERMINED THAT THE LEAK WAS ON THE PIG LAUNCHER. THE LAUNCHER WAS ISOLATED AND BLOWN DOWN. THE O-RING ON THE LAUNCHER DOOR HAD FAILED. THE O-RING WAS REPLACED. IN ADDITION, THE O-RING AT THE RECEIVER WAS REPLACED.
