Incident Narrative
A VALVE PACKING BOLT BROKE CAUSING PROPANE TO BE RELEASED FROM AROUND THE VALVE PACKING. AN AREA MONITOR PICKED UP THE RELEASE AND SHUTDOWN THE PUMPS. THE PIPELINE TECHNICIAN CONFIRMED THE VALVE PACKING LEAK WHEN HE ARRIVED AT THE STATION, AND BYPASSED THE STATION VALVE.
