Incident Narrative
CREW WAS ON SITE INVESTIGATING A GRADE 2 LEAK. WHILE CONDUCTING THE LEAK ANALYSIS AND TRYING TO PINPOINT THE LEAK'S LOCATION, A DAMAGE OCCURRED BY PROBING THROUGH THE 3"" PLASTIC MAIN CREATING A GRADE 1 LEAK. THE CREW BEGAN TO EXCAVATE TO MAKE REPAIRS ON THE GRADE 1 LEAK. AT THIS TIME, THE CREW DID NOT FOLLOW ESTABLISHED PROCEDURES WHILE ATTEMPTING TO MAKE AN UNAUTHORIZED REPAIR BY INSTALLING A MECHANICAL ""TEE"" ON AN UNCONTROLLED RELEASE OF GAS ON THE MAIN. THE GAS IGNITED CAUSING BURN INJURIES TO 2 EMPLOYEES. ONE OF THE EMPLOYEE'S INJURIES REQUIRED OVERNIGHT HOSPITALIZATION. THE GRADE 1 LEAK WAS REPAIRED THE SAME DAY. THE GRADE 2 LEAK WAS REPAIRED AT A LATER DATE.
