Incident Narrative
ON DECEMBER 2, 2016 AT APPROXIMATELY 05:48 PIPELINE CONTROL RECEIVED A HIGH GAS SHUTDOWN ON THE P2 PUMP AT FORDYCE STATION. TECHNICIAN ARRIVED ON SITE AND DISCOVERED A RELEASE OF APPROXIMATELY .4 BARRELS OF DIESEL ON THE GROUND BELOW THE PUMP. THE CAUSE OF THE LEAK WAS DETERMINED TO BE A LOOSE FITTING ON TUBING CONNECTED TO THE PUMP. THE AREA WAS CLEANED UP, THE FITTING WAS REPLACED, AND THE PUMP WAS RETURNED TO NORMAL OPERATIONS.
