Incident Narrative
A THIRD PARTY MOTORIST CONTACTED NUSTAR PERSONNEL REGARDING A CRUDE OIL ODOR AT NUSTAR'S GROOM PUMP STATION. NUSTAR IMMEDIATELY RESPONDED TO THE FACILITY AND SHUT DOWN THE PIPELINE SYSTEM. THE LEAK OCCURRED ON A LAUNCHER TRAP VALVE FLANGE, AND WAS REPAIRED BY INSTALLING A CLAMP ON THE FLANGE. UPDATE: THE CAUSE OF THE LEAK WAS FAILURE OF THE GASKET'S STEEL WINDING STRUCTURE.
