Incident Narrative
"CONTRACTOR, COUGAR CONTRACTING LLC, DAMAGED PEOPLES GAS' (PGS') 8"" STEEL HIGH PRESSURE MAIN DURING EXCAVATION. CONTRACTOR FAILED TO OBTAIN A LOCATE REQUEST FOR THE EXCAVATION TIME FRAME. LAST LOCATE REQUEST HAD BEEN OBTAINED 60 DAYS PRIOR AND WAS EXPIRED. TO MAKE THE REPAIR, A STYLE 220 REINFORCING SLEEVE WAS WELDED OVER THE DAMAGE LOCATION TO STOP THE FLOW OF GAS. PGS THEN APPLIED AN F1 COATING TO THE AREA THEN THE SLEEVE WAS X-RAYED. PGS THEN CONDUCTED A MAG-PARTICLE FILLET WELD TEST AND AN ULTRASONIC TEST."
