Incident Narrative

AT APPROXIMATELY 01:00 EMPLOYEE FOUND APPROXIMATELY 1/2 BBL OF CRUDE OIL IN THE DITCH IN FRONT OF THE DOE MANIFOLD. THE SOURCE OF THE LEAK WAS COMING FROM A WEEPING VALVE STEM AT WILLY JCT. THE STEM PACKING ON THE VALVE APPEARS TO BE FAILING DUE DAMAGE TO THE STEM. WITH HEAVY RAINS OCCURRING AT THE TIME THE 1/2 BBL MOVED INTO THE DITCH. SPILL BOOM & PADS WERE IMMEDIATELY DEPLOYED AND A VACUUM TRUCK AND OSRO CONTRACTOR DISPATCHED FOR CLEANUP. VALVE IS MONITORED AND DOWN TIME BEING SCHEDULED FOR PERMANENT REPAIR.
