Incident Narrative
ON 5/8/2020 A SMALL PACKING LEAK WAS DISCOVERED ON A 12"" MOV VALVE BY AN KINDER MORGAN EMPLOYEE. THE VALVE WAS REPACKED BUT IT CONTINUED TO SEEP PRODUCT. THE VALVE WAS SUBSEQUENTLY REPLACED A NEW VALVE.
