Incident Narrative
"APPROXIMATELY 35 GALLONS OF CRUDE OIL WERE RELEASED FROM TANK 1014B TANK HEADER PIPING AT PATOKA STATION. THE DATE AND TIME OF THE RELEASE (PART A Q4) IS INDICATIVE OF THE EARLIEST POSSIBLE TIME THE RELEASE COULD HAVE STARTED, AS IT WAS THE LAST TIME THE INCIDENT LOCATION HAD BEEN VISITED AND CONFIRMED TO BE NO RELEASE OF CRUDE OIL. A SUPPLEMENTAL REPORT WILL BE SUBMITTED PENDING FURTHER INVESTIGATION INTO THE CAUSE OF THE INCIDENT. SUPPLEMENTAL REPORT: THE CAUSE OF THE RELEASE IS ATTRIBUTED TO INTERNAL CORROSION AS A RESULT OF MIC. TO PREVENT FUTURE INCIDENTS, PLAINS PERFORMED A LOW-FLOW/NO-FLOW ASSESSMENT AND INSTALLED ISOLATIONS IN CERTAIN AREAS TO DRAIN FLUID WHEN TANKS ARE SHUT IN."
