Incident Narrative

ON 8/31/2020, A KINDER MORGAN OPERATOR DISCOVERED A LEAK ON A STOPPLE FITTING FLANGE AT THE POQUITA STATION. THE OPERATOR NOTIFIED THE KM MANAGER WHO THEN NOTIFIED THE CONTROL ROOM. KM OPERATIONS TEAM BLEW DOWN THE SPACE BETWEEN THE COMPLETION PLUG AND BLIND FLANGE USING A 1/4"" VALVE. THE BLIND FLANGE WAS THEN RE-TORQUED TO A HIGHER TORQUE VALUE TO STOP THE LEAK. AS A RESULT OF OUR ROOT CAUSE ANALYSIS, TWO NEW PROCEDURES ARE BEING DEVELOPED TO BE INCLUDED AS CO2 CONSTRUCTION STANDARDS. IN ADDITION TO THE NEW PROCEDURES, A LISTING WILL BE CREATED THAT WILL INCLUDE LOCATION AND MAINTENANCE REQUIREMENTS FOR IDENTIFIED FLANGES AND WILL BE MAINTAINED AS A SITE SPECIFIC PROCEDURE.
