Incident Narrative
CONTRACTOR, GAMEZ PROFESSIONAL DRILLING, INC., DAMAGED PEOPLES GAS' (PGS') 4' PLASTIC GAS MAIN DURING INSTALLATION OF A COMMUNICATIONS CABLE. THE CONTRACTOR REQUESTED AND OBTAINED A LOCATE FOR THE EXCAVATION, THE GAS PIPELINE WAS CORRECTLY MARKED, AND THE LOCATE MARKS WERE VISIBLE IN THE AREA. THE GAS MAIN WAS DAMAGED DURING THE 3RD PARTY'S DIRECTIONAL DRILLING PROCESS. THE RESULTING GAS LEAK WAS MADE SAFE BY SQUEEZING THE 4' PLASTIC GAS MAIN AND WAS REPAIRED BY INSTALLING A NEW 4' PLASTIC PIPE. THE PIPE HAD A LEAKAGE RESURVEY PERFORMED AFTER THE REPAIR ON 06/19/19.
