Incident Narrative
A VICTAULIC CLAMP ON A 2-INCH DRAIN LINE CONNECTED TO A STRAINER IMMEDIATELY UPSTREAM OF METER M-1700 FAILED, RESULTING IN A CRUDE OIL SPILL. A 1-INCH DRAIN LINE WAS OPENED TO THE SUMP DURING MAINTENANCE AND WAS NOT CLOSED AFTER THE MAINTENANCE WAS COMPLETED. STATIC PIPELINE PRESSURE ON THE DRAIN LINE CAUSED A VICTAULIC CLAMP TO FAIL. THE CLAMP FAILURE IS THE ORIGIN OF THE SPILL.
