Incident Narrative
DURING NORMAL OPERATIONS, AN OPERATOR DISCOVERED PRODUCT FLOATING ON TOP OF RAINWATER IN THE TANK DIKE AREA UNDERNEATH A MANIFOLD VALVE. OPERATOR VERIFIED THE LEAK WAS NOT ACTIVE AT THE TIME OF DISCOVERY. THE LEAK OCCURRED DURING THE TRANSIT (OPENING/CLOSING) OF THE VALVE. OPERATOR IMMEDIATELY NOTIFIED RESPONSE PERSONNEL AND SUPERVISION AND BEGAN REMEDIATION ACTIVITIES. PRODUCT DID NOT COME INTO CONTACT WITH ANY EQUIPMENT OR WATERWAY.
