Incident Narrative
ON 9/16/2021 AT 9:30 AM OPERATIONS DISCOVERED CRUDE IN THE PHASE 3 MANIFOLD AT APPELT TERMINAL. THE MANIFOLD WAS ISOLATED AND THE FLOW WAS REROUTED. FIELD EXAMINATION DETERMINED THAT THE LEAK OCCURRED DUE TO INTERNAL CORROSION OF THE PIPE. A PLIDCO CLAMP WAS INSTALLED TO REPAIR THE PIPE. ENTERPRISE NOTIFIED TRRC AT 10:39 ONCE THE VOLUME WAS ESTIMATED TO EXCEED 5 GALLONS. AT 11:02, ENTERPRISE ESTIMATED THE COST OF THE REPAIR WOULD EXCEED $50K AND THE NRC WAS IMMEDIATELY NOTIFIED.
