Incident Narrative
ON FEBRUARY 11, 2022 AT 12:13, ENTERPRISE HOUSTON CONTROL CENTER (HCC) RECEIVED A CALL FROM THE SHELL UGS FACILITY OF A HAZARDOUS GAS ALARM AND OBSERVED A GAS CLOUD FORMING NEAR THE ENTERPRISE PUMP. SHELL DETERMINED THE POINT OF THE LEAK TO BE AT THE ENTERPRISE PUMP AND NOTIFIED ENTERPRISE HCC AT 12:22. ENTERPRISE HCC REMOTELY ISOLATED THE PUMP UPSTREAM AND DOWNSTREAM BY 12:40. ENTERPRISE TECHNICIAN ARRIVED ONSITE AT 13:05 AND STOOD BY AS SHELL CONTINUED THEIR EMERGENCY RESPONSE ACTIVITY. ONCE IT WAS DETERMINED TO BE SAFE TO ENTER, ENTERPRISE VENTED REMAINING CASE PRESSURE TO FLARE AT 17:05. THE PUMP REMAINED SHUT DOWN AND ISOLATED UNTIL FEBRUARY 16. ON FEBRUARY 16, IT WAS DISCOVERED THAT A SECONDARY SEAL FAILURE HAD OCCURRED AND THE ANTICIPATED REPAIR COST WOULD EXCEED $50K. AS A RESULT, THE NRC WAS IMMEDIATELY NOTIFIED. ENTERPRISE INVESTIGATED THE SEAL FAILURE AND DETERMINED THE DRIVE END SEAL FACES, INBOARD STATIONARY RETAINER AND GLAND ADAPTER SEAL SHIFTED AXIALLY. THIS SHIFT RESULTED IN ENOUGH COMPRESSION AND TORQUE INSIDE THE SEAL TO SHEAR THE ANTI-ROTATION PIN OFF THE GLAND ADAPTER ALLOWING THE RETAINER TO SPIN. THE NON-DRIVE END SEAL APPEARS TO HAVE SHIFTED AS WELL BASED ON THE MARKINGS ON THE SHAFT AND CONDITION OF THE FACES. WITH BOTH SEALS SHIFTING AWAY FROM THE PUMP, IT SUGGESTS THE SET SCREWS WERE NOT CORRECTLY INSTALLED. THE SET SCREWS HAD SIGNIFICANT WEAR ON THEM WHICH LIKELY MEANS THEY WERE SET DOWN INTO THE SHAFT, BUT POSSIBLY NOT FULLY TORQUED. THE RADIAL WEAR ON THE SLEEVE AND COLLAR INDICATES BOTH SEALS SPUN ON THE SHAFT, BUT THIS MAY HAVE BEEN SECONDARY TO THE AXIAL MOVEMENT ONCE THE SET SCREWS SLIPPED AND WERE WORN. PART A QUESTION 17B INDICATES THE RESTART OF THE PIPELINE TO NORMAL OPERATIONS WITH THE PUMP CONTINUING TO BE PHYSICALLY DISCONNECTED FROM THE MAINLINE. THE PUMP REMAINS OUT OF SERVICE AND ALL NECESSARY REPAIRS WILL BE COMPLETED IF THE PUMP IS PLACED BACK INTO SERVICE. THE PHMSA ACCIDENT DIVISION WAS NOTIFIED ON 2/16/2022 AT 12:15. THE 48-HOUR UPDATE TO THE NRC TOOK PLACE ON 2/18/2022 AT 10:18.
